Case Management Services

Cape Senior Services is staffed with social workers who understand the aging process and disease progression. They are familiar with community agencies and long-term care facilities. They will work with each family to develop a plan of care for their loved one. 

When You Need Help

Step One: The caregiver reaches out for help from Cape Senior Services and a case manager will set up an initial consultation.

Step Two: The case manager will meet the senior in their home to get to know them and their routine and meet with the caregiver to discuss their goals.

Step Three: The case manager will meet with the caregiver(s) to discuss the plan of care. A care plan identifies the needs of the senior and details the type of support that will be provided. Once the family has agreed to the plan of care, Cape Senior Services will match them with a caregiver(s). 

Step Four: Services begin. Case managers will continue to provide support to the caregiver.

When Needs Change

Cape Senior Services case managers will remain available to the caregiver and be their “point person”. In the event that the disease progression is such that our caregiving services are no longer appropriate, the case manager will continue to work with the family on the possible next steps. 

Our hope for every family is that their loved one can age in place. However, sometimes it is in the best interest of the senior to obtain additional services that may include a long-term care facility. IF that need arises, the case manager will educate the family on what other services might be beneficial and/or assist them with any decisions they make about transitions. We understand that these decisions are very tough and we will be with you every step of the way.
